Client Intakes
Client intake forms are to be filled out honestly and completely. The information provided help to ensure that your session is safe, effective, and tailored to suit your needs. It is your responsibility to inform your massage therapist of any medical conditions, injuries, medications, allergies, or changes in your health, both now and in the future. There are separate intake forms for minors, prenatal, and postpartum clients.
Cancellation & Late Policy
Your appointment time is reserved especially for you. To provide the best care and to respect both my time and other clients, the following policy applies:
Cancellation & Rescheduling
I require at least 24 hours’ notice to cancel or reschedule an appointment.
- Appointments cancelled with less than 24 hours’ notice will be charged 50% of the service fee.
- Same day bookings are considered final and cannot be cancelled or rescheduled. Same day cancellations or missed appointments will be charged 100% of the service fee.
- No shows (missed appointments without notice) will be charged 100% of the service fee.
- Three no shows may result in non-rebooking.
Late Arrivals
If you arrive late, your session will be shortened to avoid impacting the next client. The full service fee will still apply. If you are more than 15 minutes late, the appointment may be considered a no-show and charged accordingly.
Emergencies
I understand that emergencies and unexpected situations happen. Please contact me as soon as possible, and exceptions may be made at my discretion.

I understand that massage therapy is professional and non-sexual in nature. Any inappropriate behavior or requests will result in immediate termination of the session, and full payment will still apply.

Consent for Minors (under 18)
I understand that clients under the age of 18 must have consent from a parent or legal guardian to receive massage therapy.
As the parent or legal guardian, I consent to the massage therapy for the minor named below. I confirm that I am legally authorized to provide consent and the the health information for the minor is accurate and complete.
I understand that:
- A parent or legal guardian must be present in the treatment room or on the premises during the session, in accordance with business policy and applicable law.
- Massage therapy is professional and non-sexual in nature
- I am responsible for communicating medical conditions, injuries, medication, or changes in the minor’s health
- Certain areas of the body may be excluded from treatment for minors
- I may withdraw consent at any time by notifying the therapist
Forms
Intake and consent forms must be completed prior to your session. Please come 15 minutes early to your first session to complete the forms or you may print out and complete the forms before arriving.
