Client intake forms are to be filled out honestly and completely. The information provided help to ensure that your session is safe, effective, and tailored to suit your needs. It is your responsibility to inform your massage therapist of any medical conditions, injuries, medications, allergies, or changes in your health, both now and in the future. There are separate intake forms for minors, prenatal, and postpartum clients.
Your appointment time is reserved especially for you. To provide the best care and to respect both my time and other clients, the following policy applies:
I require at least 24 hours’ notice to cancel or reschedule an appointment.
If you arrive late, your session will be shortened to avoid impacting the next client. The full service fee will still apply. If you are more than 15 minutes late, the appointment may be considered a no-show and charged accordingly.
I understand that emergencies and unexpected situations happen. Please contact me as soon as possible, and exceptions may be made at my discretion.
